Gather the following; 

1.  Invoice (s) or aging report

2.  Any agreement/contract/credit application you might have with your debtor 

STEP 3

 Fill out the placement form as best as possible and fax to Solberg & Kennedy along with STEP 2 back up material.  Fax number is 602-952-9766.

Solberg & Kennedy will fax an Acknowledgement Report back to you.  They will follow up with a phone call.
